People need to be able to refuse medical treatment. Period. It's nobody's business but their own. At the same time, it's reasonable for a health facility to insist that employees able to receive a treatment should receive it for the safety and peace of mind of the patients. Think of Typhoid Mary, who was a cook. She was a carrier. At the time, germs and such-like weren't well understood so she kept going to cook for families, leaving when typhoid took hold. Sad story - she was isolated for the last two decades of her life because she wouldn't give samples and because people kept getting typhoid, some dying, when she worked for them. en.wikipedia.org/wiki/Mary_Mallon
